There was no certified food safety manager or CFSM on duty as required. Ensure a department registered CFSM is on duty any time this facility is open or operating. Register additional employees to ensure all shift are covered by a CFSM.

Inspector Comments
The sanitizer dispenser was not dispensing the appropriate concentration of sanitizer. Quaternary sanitizer must reach a concentration of 200ppm. Have this dispenser serviced and send the service report to the Department. In the mean time, utilize a chlorine sanitizer for ware washing and ensure employees are regularly using sanitizer test strips to monitor concentrations.

Ambient temperature in the self-service hot-holding case was measured at 158°F. With doors continually being opened, food within was measured at 128°F. Individually boxed pizza sliced, labeled as placed into hot holding at 10:13 am, were past the 4-hour time control expiration. Affected products were discarded. Chicken wings and boneless chicken products were placed into unit at 1:40 pm but not properly labeled. Label printed during inspection to cover all remaining products within.

1. Mouse droppings observed on floor behind soda syrup packages. 2. Fruit flies present in and around floor drain near soda syrup packages. Remove and discard droppings. Contact facility’s exterminator with findings and for additional monitoring and/or treatment.

Bottom of wall area near electrical panel has deteriorated and cove base has separated from wall. Repair affected area to provide surfaces which are smooth, easily cleanable and non-absorbent.

1. Floor drain near the soda syrup storage/supply lines contains an accumulation of a slimy, mold-like substance. 2. Floors below soda syrup contain a standing, sticky liquid (presumably soda/beverage syrup). Thoroughly clean affected areas and maintain.

A black, mold-like slimy substance is present in the ice chute of the self-service beverage dispenser. Affected equipment was detail cleaned and sanitized during inspection.

1. Steak & Cheese Taquito (118°F), Buffalo chicken roller (131 °F), cheeseburger Big Bite (113 °F) at the hot-holding rollers, were measured below range for hot-holding. 2. Pizza (121 °F) and chicken wings (118 °F) in the hot-holding case were measured below range for hot holding. Hot food must be held at 135 °F and above. Affected products were reheated during inspection and placed back into hot-holding units. Temperature settings on hot-holding rollers were also adjusted during inspection.

Self-service dispensing tongs at the hot-holding rollers are untethered, in plastic cups. Affix tethers to tongs to prevent contamination.
