Inspector Comments
Ambient temperature in the self-service hot-holding case was measured at 158°F. With doors continually being opened, food within was measured at 128°F. Individually boxed pizza sliced, labeled as placed into hot holding at 10:13 am, were past the 4-hour time control expiration.
Affected products were discarded.
Chicken wings and boneless chicken products were placed into unit at 1:40 pm but not properly labeled.
Label printed during inspection to cover all remaining products within.

1. Mouse droppings observed on floor behind soda syrup packages.
2. Fruit flies present in and around floor drain near soda syrup packages.
Remove and discard droppings. Contact facility’s exterminator with findings and for additional monitoring and/or treatment.

1. Floor drain near the soda syrup storage/supply lines contains an accumulation of a slimy, mold-like substance.
2. Floors below soda syrup contain a standing, sticky liquid (presumably soda/beverage syrup).
Thoroughly clean affected areas and maintain.
