Food safety manager certification has expired for facility owner. A CFSM must be on duty at all times the facility is open and/or food is being prepared. A completed application(SA-71)for CFSM, or proof of enrollment in an approved safety class must be submitted to the Department. Registration completed for ServSafe Food Safety Manager course and exam.

Wall-mounted Quaternary Ammonium Compounds dispenser at the 3-compartment sink is not providing adequate sanitizer concentration. Facility is to use Quat-based sanitizer tablets until dispenser is assessed and repaired.

Inspector Comments
Mouse droppings observed on floor behind the Galaxy chest freezer. Remove and discard droppings to effectively monitor activity. Clean affected area. Contact facility’s exterminator with findings and for additional monitoring and/or treatment.

A leak is present at the overflow valve of the hot water heater. Sufficient hot water available in facility. Repair or replace affected plumbing fixture(s) to ensure adequate hot water temperature and availability.

Hot water temperature at the low-heat mechanical dishwasher was measured between 105°F and 110°F. Hot water heater was adjusted during inspection. Water temperature at the mechanical dishwasher was raised to 120°F for wash and rinse.

Low-heat mechanical dishwasher not providing adequate sanitizing solution concentration. Approximately 10 ppm of Chlorine-based sanitizer was measured through several cycles of unit. Sanitizer container was replaced with new. Unit was cycled and 100 ppm Chlorine was then measured.

Facility has placed a heavy mat above the section of damaged floor in the walk-in cooler. Per owner, a plate will be added to repair affected area.
