Inspector Comments
There is no food safety manager certified by this Department (CFSM) on duty as required. A CFSM must be on duty at all times the facility is open and/or food is being prepared. A completed application (SA-71) for CFSM, or proof of enrollment in an approved safety class must be submitted to the Department.

Proper eating, tasting, drinking, or tobacco use
Inspector Comments
Upon arrival, observed food employee eating in front of the Main Prep Unit in the kitchen. An employee may not eat, drink, or use any form of tobacco in areas where the contamination of exposed food, clean equipment, utensils, linens, etc, can result. Keep all food and drink in designated areas only.

Food properly labeled; original container
Inspector Comments
Facility is reusing single-use food containers for in-house food prep and storage. Once a food container’s food product has been used or emptied, that original container shall not be used for other food storage. Facility must use a food-safe container that can be cleaned and sanitized.
