Two spay bottles of window cleaner were hanging from the dry storage rack. Poisonous or toxic materials shall be stored and/or displayed for retail sale so they cannot contaminate food, equipment, utensils, linens and single-service and single-use articles by one or both of the following:
(1) Separating the poisonous or toxic materials by spacing or partitioning.
(2) Locating the poisonous or toxic materials in an area that is not above food, equipment, utensils, linens and single-service or single-use articles.
Theses articles were relocated to under the 3-compartment sink.

Cutting knife was being stored on the prep surface of the Grill Prep unit but the unsanitized lid of the prep unit was closed and touching the food surface portion of the knife. During pauses in food preparation or dispensing, food utensils may be stored on the clean portion of the food preparation table or cooking equipment only if the in-use utensil and the table or equipment are cleaned and sanitized frequently. This item was removed and placed in the warewashing area.? Facility uses magnetic strips throughout. Suggested installing one around the prep area.

Employee energy drink was found in the double stand up refirgerator. An employee may not eat, drink or use any form of tobacco unless the use occurs in designated areas where the contamination of exposed food; clean equipment, utensils and linens; unwrapped single-service and single-use articles; or other items needing protection cannot result. The can was relocated.

The tips of the condement squeeze bottles stored in the top portion of the grill prep unit is touching the upper, inside lid of the prep unit, leaving food residue on the lid. Relocate/rearrange the squeeze bottles, clean/sanitize the lid and the lids to the wares.

The door to the walk-in freezer is showing signs of heavy thaw and re-freeze, and the gasket is shredded. Equipment components such as doors, seals, hinges, fasteners and kick plates shall be kept intact, tight and adjusted in accordance with manufacturer's specifications. A new door has been ordered. Facility is waiting on delivery and installation.

Front Prep Unit was measureing at 50 Degrees F, and all foods held inside and on top were measured with temperature ranges from 50 to 52 Degrees F. All Time/temperature control for safety (TCS) food must be cold held at or below 41ºF.
All food held in storage was moved to the walk-in cooler, and all TCS items up top were discarded.

This container was discarded.

There is no food safety manager certified by this Department (CFSM) on duty as required. A CFSM must be on duty at all times the facility is open and/or food is being prepared. A completed application(SA-71)for CFSM, or proof of enrollment in an approved safety class must be submitted to the Department. Manager, Larry Arinsberg, obtained his ServSafe Certificate. Immediately apply for BCDH CFSM.

Food employee observed cracking eggs and returning to food preparation without removing gloves and washing hands. Food employees shall clean their hands and exposed portions of their arms as specified in § 131 (relating to cleanliness of hands and exposed portions of arms) as follows:
(1) Immediately before engaging in food preparation activities such as working with exposed food, working with clean equipment and utensils, and working with unwrapped single-service and single-use articles.
(2) After touching bare human body parts other than clean hands and clean, exposed portions of arms.
(3) After using the toilet room.
(4) After caring for or handling service animals or aquatic animals as specified in § 153(b) (relating to animals).
(5) Except as specified in § 151(a)(2) (relating to food contamination prevention), after coughing, sneezing, using a handkerchief or disposable tissue, using tobacco, eating or drinking.
(6) After handling soiled equipment or utensils.
(7) During food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing tasks.

Manager reviewed glove use and handwashing procedure during inspection. Continue to monitor.

Grab and go re-packaged food products including cookies, parfait and various muffins are not labeled with ingredients or weight.
Label information on packaged foods shall include the following:
(1) The common name of the food, or absent a common name, an adequately descriptive identity statement.
(2) If made from two or more ingredients, a list of ingredients in descending order of predominance by weight, including a declaration of artificial color or flavor and chemical preservatives, if contained in the food.
(3) An accurate declaration of the quantity of contents.
(4) The name and place of business of the manufacturer, packer or distributor.
Manager will develop a binder to include each food product, weight, ingredients and manufacturer to provide to customers. Limited re-packaged food products - items will not be individually labeled but will include common name of product.
