Notify pest control of recent activity and have out to treat on a more frequent basis until the activity is eliminated. Facility must be completely detail cleaned eliminating all conditions condusive to pest harborage, food & water.
41	In-use utensils: properly stored Corrected On-Site. Inspector Comments Observed plastic scoop in the flour bin with handle buried. Remove scoop clean and sanitize. Scoop handle must not come into contact with food products and should be properly stored.
46	Warewashing facilities: installed, maintained, & used; test strips Inspector Comments A test kit or other device that accurately measures the concentration in mg/L or ppm of sanitizing solutions shall be provided. Test strips were unavailable at the time of inspection. Order test strips for the 3 bay sink prior to cleaning and sanitizing utensils, etc.
47	Non-food contact surfaces clean Corrected On-Site. Inspector Comments Observed accumulation of rice spilled underneath the shelving in the dry storage room. Pull shelving out, clean and sanitize.

8	Adequate handwashing facilities supplied & accessible Inspector Comments The cold water main valve of the handwashing sink was turned off upon arrival. Cold water knob was turned on during inspection. Ensure hot and cold water are provided at handwashing sinks at all times.
14	Food-contact surfaces: cleaned & sanitized Inspector Comments 1. The dough mixer contains food debris on the underside and arm. 2. The bulk flour and sugar bins on cook line contain a build up of food debris. Clean and sanitize to be free of visible debris.

There are multiple drain flies in the food prep sink in the kitchen, contact pest control operator to minimize activity. 2. There are air gaps at the base of both sets of doors in the cafeteria area. Provide new weather stripping to provide air tight seal.
45	Food & non-food contact surfaces cleanable, properly designed, constructed, & used Inspector Comments There is a concrete trap in the floor of the kitchen containing large gaps were the grease trap is located. Provide cleanable, non-porous cover for trap.
49	Plumbing installed; proper backflow devices Inspector Comments The mop sink closet contains an elevated drain and unfinished flooring. Provide plumbed mop sink above drain for proper disposal of waste water.
