Inspector Comments
There is no food safety manager certified by this Department (CFSM) on duty as required. A CFSM must be on duty at all times the facility is open and/or food is being prepared. A completed application(SA-71)for CFSM, or proof of enrollment in an approved safety class must be submitted to the Department.
8 Adequate handwashing facilities supplied & accessible

Inspector Comments
1. The handwashing sink in the front prep area was blocked with items upon arrival. 2. The handwashing sink in the ware washing area has no paper towels. Each handwashing sink shall be provided with a supply of hand cleaning liquid, powder or bar soap, individual, disposable towels, a continuous towel system that supplies the user with a clean towel or a heated-air hand drying device. Ensure handwashing sinks are accessible for use at all times.
14 Food-contact surfaces: cleaned & sanitized

Inspector Comments
The back prep table in ware washing room contains dirty wares and food. Clean and sanitize area to be free of visible debris.
20 Proper cold holding temperature

Inspector Comments
The dessert display case was reading an ambient temperature of 47 degrees F. Food items in the unit measure between 44-47 degrees. Affected items were relocated during inspection and service ticket was placed during inspection.
53 Physical facilities installed, maintained, & clean

Inspector Comments
There are many missing tiles from the mop sink enclosure and wall is in poor repair. Repair to be a smooth cleanable surface.

1. Handwashing sink in the warewashing room is blocked with several storage crates. A handwashing facility shall be maintained so that it is accessible at all times for employee use. Affected items were relocated during inspection, allowing sink to be accessible. Corrected on-site. 2. No paper towels at the hand wash sink in the warewashing area. Each handwashing sink shall be provided with a supply of hand cleaning liquid, powder or bar soap, individual, disposable towels, a continuous towel system that supplies the user with a clean towel or a heated-air hand drying device.
36 Insects, rodents & animals not present; no unauthorized persons

Inspector Comments
Fruit flies observed around the mop sink and in cabinetry below the dump sink. Keep floor drains and affected areas free of food residue and debris. Contact facility’s exterminator with findings and for additional monitoring and/or treatment.
49 Plumbing installed; proper backflow devices

Inspector Comments
1. A leak is present at the wall-mounted chemical dispenser above the mop sink. 2. Standing water is present below the drain line at the dump sink.. Repair or replace affected plumbing fixture(s) to ensure proper operation and to reduce conditions conducive to pest activity.

Inspector Comments
Between-use food-contact utensils found being stored in bucket of sanitizing solution. In-use utensils and between-use storage. (a) General. During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored in the food with their handles above the top of the food and the container, unless the food is non-potentially hazardous and within containers or equipment that can be closed, such as bins of sugar, flour or cinnamon--in which case the food preparation and dispensing utensils shall be stored with their handles above the top of the food within the containers. (b) Storage on cleaned and sanitized tables or equipment. During pauses in food preparation or dispensing, food preparation and dispensing utensils may be stored on a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the table or equipment are cleaned and sanitized at required frequency. (c) Storage in running water. During pauses in food preparation or dispensing, food preparation and dispensing utensils may be stored in running water of sufficient velocity to flush particulates to the drain, if used with moist food such as ice cream or mashed potatoes. (d) Storage in clean, protected locations. During pauses in food preparation or dispensing, food preparation and dispensing utensils may be stored in a clean, protected location if the utensils, such as scoops, are used only with a food that is not potentially hazardous. (e) Storage in a container of water. During pauses in food preparation or dispensing, food preparation and dispensing utensils may be stored in a container of water if the water is maintained at a temperature of at least 135°F and the container is cleaned at required frequency.  Affected utensils were relocated during inspection.
45 Food & non-food contact surfaces cleanable, properly designed, constructed, & used

Inspector Comments
Thick layer of ice accumulation is present at the bottom interior surface in the True 2-door "savory" freezer. Remove ice and maintain. Assess unit to determine cause of leak/ice build-up. Per CFSM, a work order as been placed for repair of this freezer unit.
46 Warewashing facilities: installed, maintained, & used; test strips

Inspector Comments
The digital display on the high-temperature sanitizing mechanical dishwasher is not functioning. Machine cannot be properly cycled, nor wash/final rinse temperatures determined. All wares must be washed and sanitized using the 3-compartment sink. CFSM confirmed that a service call has been placed for assessment/repair of this unit.
