Inspector Comments
Lexan pans were found cycled through the mechanical warewashing machine with labels still affixed. Once pans are stacked/nested, labels then touch the food-contact surface of the adjacent container. All labels and label adhesive must be removed from unclean wares prior to warewashing. CFSM removed affected labels and moved affected wares to be re-washed, during inspection.
17 Proper reheating procedures for hot holding

Inspector Comments
Soup in a hot-holding crock was measured at 124°F. Potentially hazardous food that is cooked, cooled and reheated for hot holding must be reheated so that all parts of the food reach a temperature of at least 165°F for 15 seconds. Reheating for hot holding must be done rapidly and the time the food is between cold-holding temperature and 165°F may not exceed 2 hours. Soup was reheated during inspection and subsequently re-measured at 166°F.

1. Handwashing sink at the front of the cook-line is out-of-use, awaiting a drain line replacement. CFSM showed work order placed for repair. 2. No soap is available at the hand wash sink in the server area. Each handwashing sink shall be provided with a supply of hand cleaning liquid, powder or bar soap, individual, disposable towels, a continuous towel system that supplies the user with a clean towel or a heated-air hand drying device. Replacement soap dispenser arrived and was installed during inspection. Corrected on-site.
14 Food-contact surfaces: cleaned & sanitized

Inspector Comments
Quaternary ammonium compounds sanitizer dispenser at the bar area 3-compartment sink is not providing sanitizer at proper volume/concentration. Sanitizing solution was measured between 0 and 150 ppm. Bar staff was instructed to reduce the water used into this sink to provide a proper sanitizing solution concentration (or to fill sink using DDBSA/Lactic acid sanitizing solution available in the kitchen.

Pre-portioned bags of shrimp on the cookline prep area are not labeled to indicate a time by which they are to be sold, or discarded. If time, rather than temperature, is used as the public health control the food must be marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food is removed from temperature control. Restaurant opened at 11 am. Affected product to be used or discarded by 3 pm.

Several reduced-oxygen, individually-packaged salmon filets were found being thawed under refrigeration, sealed. Handling instructions note to keep frozen and open packaging prior to thawing under refrigeration. Introducing oxygen to product prior to thawing under refrigeration eliminates conditions (temperatures above 38ºF and without oxygen) conducive to the growth of Clostridium botulinum type E. Affected product was measured at 34 ºF. Packages were opened during inspection to eliminate reduced-oxygen conditions, and allow for proper thawing.

Fruit flies are present on the walls above the blender area of the bar. Contact facility’s exterminator with findings and for additional monitoring and/or treatment. Standing water is present in the bar floor drain. Ensure that all drains are clean and clear of any food debris/standing water/obstructions to reduce conditions conducive to pest activity.

Inspector Comments
Initial cycles of the high-heat sanitizing mechanical dishwasher produced a maximum final rinse water temperature of 118°F. Tripped circuit breaker, on which the unit's hot water heater booster resided, was re-set during inspection. Subsequent cycles of this unit produced wash water temperature of 149°F and final rinse water temperature of 189°F.
20 Proper cold holding temperature

Fish (45°F) and shrimp (46°F) in the cook-line refrigerated drawers (top) were measured above range for cold-holding temperature. Time/temperature control for safety (TCS) food must be cold held at or below 41ºF. Significant ice accumulation is present on the unit's interior wall behind the top drawer. Items in the bottom drawer were measured within proper cold-holding range of 41°F and below. Per CFSM, unit will be defrosted today.

Tuna, shrimp and diced tomatoes in the 'fry' cook-line prep unit were measured at 48°F. Staff measured temperature of all Time/Temperature Control for Safety (TCS) foods in unit during inspection. Time/temperature control for safety (TCS) food must be cold held at or below 41ºF. All affected products were relocated to the walk-in. Prep unit must be placed out-of-servive until repaired and properly holding temperature at 41°F and below, or replaced. Contact this Department for equipment approval.

Contact facility's exterminator with findings and for additional monitoring and/or treatment.

Drain from handwashing sink is not properly aligned with the floor drain below and is emptying partially onto the floor. Reposition to direct wastewater into floor drain.

Ecolab Sink & Surface Sanitizer (DDBSA/Lactic Acid) in a Sanitizing solution container, was measured above range (272-700 ppm)for DDBSA concentration. Level measured was >848 ppm. Facility must use Quat-based sanitizer dispenser at the bar sink to fill sanitizer buckets until DDBSA/Lactic Acid dispenser is serviced and providing sanitizer with proper concentration. Forward Ecolab service report to this Department.
