Inspector Comments
Multiple items in the walk-in cooler were being stored either uncovered or on the floor. All stored food items must be properly covered and store at least 6 inches off the floor to prevent possible contamination. Reorganize the walk-in and ensure all employees are aware of food storage requirements. Raw eggs were being stored in a bowl above vegetables in the prep-cooler. Ensure all employees are aware that raw egg and meat products must be stored below ready-to-eats foods to prevent possible contamination.

Prepared foods in the walk-in cooler are not being date marked as required. Date mark all prepared food items to ensure all items are being used or discarded within 7 days of preparation.
33 Approved thawing methods used

Inspector Comments
Vacuum sealed fish was observed completely thawed while still within the vacuum seal packaging. Package notes to remove from vacuum seal prior to thawing. Ensure all employees are ware that vacuum sealed fish must be removed from the packaging prior to thawing to prevent possible contamination. These items were voluntarily discarded during this inspection.

Employee observed preparing raw cucumbers for use in sushi rolls with bare hands. All Ready To Eat food must be prepared with means to eliminate contaminating from food employees hands. Gloves must be worn for all sushi preparation.

Food workers are not wearing hair restraints as required. Employees shall wear hair restraints such as hats, hair coverings or nets, that are effectively designed and worn to keep their hair from contacting exposed food; clean equipment, utensils and linens; and unwrapped single-service and single-use articles.

Inspector Comments
Server Prep Unit and Sushi Display Case are all holding improper temperatures. Server Prep has an ambient temperature of 50F. Sushi Prep has an ambient temperature of 48F, with internal temperatures of products inside at 55-58F. All Time/Temperature Control for Safety (TCS) foods must be cold held at 41F or below. All TCS products above 41F must be discarded. Must repair server prep unit and sushi display case prior to use. Sushi Display case may be used with ice baths to keep product cold for today only. Call refrigeration repair company to get units in temperature.

Food employees are not washing their hands as required before putting on gloves, after handling soiled tableware, after handling raw meat, before handling clean tableware, equipment, utensils. CFSM must review hand washing requirements with staff and train staff to utilize hand soap instead of rinsing hands with water only.

Observed kitchen hand sinks lacking paper towels and rear sink obstructed with utensils. Ensure all hand sinks remain unobstructed at all times and are supplied with soap and paper towels.

Inspector Comments
Facility lacked current parasite destruction letter from supplier for sushi fish. Submit the required documentation to this Department and retain a new copy annually on-site.

Inspector Comments
Observed nearly all food uncovered inside walk-in cooler. Provide food-grade covers for all food such as plastic wrap or lids. Raw eggs over produce in walk-in cooler. Raw eggs and meat/poultry shall be stored beneath other food.
20 Proper cold holding temperature

Inspector Comments
Observed TCS food throughout kitchen such as wontons, eggrolls, battered chicken, diced pineapple and cut produce stored above 41F. Time/temperature control for safety (TCS) food must be cold held at or below 41ºF. Instructed facility to maintain temperatures utilizing ice baths or cold-holding units. In lieu of temperature, facility may only utilize a four- hour time log if it is in writing for each item and food is discarded after maximum of four hours. Affected items were immediately moved to cold-holding.
27 Compliance with variance, specialized process, & HACCP plan

Facility is currently utilizing pH to acidify sushi rice without a HACCP plan. Facility must provide this Department with a HACCP plan for the acidification of sushi rice for review.

The following non-food contact surfaces were observed to have heavy accumulation of debris and build-up: *High-touch surfaces such as cooler handles, faucets doors. *Interior of wall-in cooler walls and ceiling. *Between and on all sides of cook’s line equipment. *Floors and walls throughout, including under and behind equipment and shelving. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. A detailed deep clean must occur and these surfaces maintened with more frequent cleaning to prevent build-up.

Touch surfaces of all equipment throughout the kitchen are unclean and sticky to touch. Must provide a deep and detailed cleaning of all surfaces of equipment with a concentration of door handles and lids.
