2 open, personal drink bottles were being stored in the same display case as the bottled drinks being sold to consumers. An employee may not eat, drink or use any form of tobacco unless the use occurs in designated areas where the contamination of exposed food; clean equipment, utensils and linens; unwrapped single-service and single-use articles; or other items needing protection cannot result. Keep all food and drink in designated areas only, or assign a spot in the refrigerator labeled for employee use only.
14 Food-contact surfaces: cleaned & sanitized

There is no sanitizer at the 3-bay sink for manual warewashing. Utensils and food-contact surfaces of equipment shall be sanitized after cleaning, and before use. For chemical operations, the application of sanitizing chemicals by immersion, manual swabbing, brushing or pressure spraying methods, using a solution of either 50-100ppm chlorine or 200-300ppm quaternary ammonia, must be completed before next use. There seems to be an issue with the mixer, call EcoLab for service. Manually add sanitizer and test concentration before use. Once serviced, submit service report to the department.

Both food handlers on separate occasions attempted to serve customers by putting on gloves without washing hands first. Food employees shall clean their hands and exposed portions of their arms as follows: (1) Immediately before engaging in food preparation activities such as working with exposed food, working with clean equipment and utensils, and working with unwrapped single-service and single-use articles. (2) After touching bare human body parts other than clean hands and clean, exposed portions of arms. (3) After using the toilet room. (4) After caring for or handling service animals or aquatic animals. (5) After coughing, sneezing, using a handkerchief or disposable tissue, using tobacco, eating or drinking. (6) After handling soiled equipment or utensils. (7) During food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing tasks. (8) When switching between working with raw food and working with ready-to-eat food. (9) Before donning gloves for working with food. (10) After engaging in other activities that contaminate the hands. Both employees disposed of the gloves they put on, and washed hands properly.

There is evidence of food dumping in the serving station (front) hand washing sink. A hand washing sink is for handwashing only, and serve no other purpose. Must use prep sink for all foods.

Several tongs are being stored above the sanitized compartment of the 3-bay sink, and some still contained food remnants. These items were removed from hanging and placed in the washing compartment to be rewashed.

Upon arrival, there was a cell phone charging right next to a cutting board and knife on the prep table. Food and wares shall only contact surfaces of equipment and utensils that are cleaned and sanitized. Phone was relocated and prep table was sanitized.

Observed food worker touch rice with the back of his hand to check the temperature. Food employees may not touch exposed, ready-to-eat food with their bare hands, and shall use suitable utensils such as deli tissue, spatulas, tongs, single-use gloves or dispensing equipment. The rice was discarded.

Upon arrival, the cook-line handwashing station was not supplied with paper towels. Each handwashing sink shall be provided with a supply of hand cleaning liquid, powder or bar soap, individual, disposable towels, a continuous towel system that supplies the user with a clean towel or a heated-air hand drying device.
22 Time as a public health control; procedures & record

Inspector Comments
Two pans of Rice sitting on a cart in front of the grill did not have a time as to when they are to be sold by or discarded. One of them did have a sticker, but it was old, and was not the correct date. If time--rather than temperature--is used as the public health control the food shall be marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food is removed from temperature control. Both pans were discarded.

Pita Triangles were sitting on top of chicken in the hot hold area. Must use separate equipment for each type or arranging each type of food in equipment so that cross contamination of one type with another is prevented. Pita triangels were discarded.

Facillity uses time as a control for the shaved meat. The pan where the shaved meat is held is not being marked with the time. Meat in this pan was temped at 83 degrees F, and no indication as to how long it was sitting out. Pan moved to the refrigerator for holding.

Inspector Comments
Knife, scoop, and meat shaver are dipped into high pans of stagnate water for cleaning bewtween uses. During pauses in food preparation or dispensing, utensils may be stored in running water of sufficient velocity to flush particulates to the drain, if used with moist food. Pans of water were removed and facility will now use time as a control.

Baked goods including cheese cake, chocolate chip cookies and baklava are re-packaged but lacking a label. Label information on packaged foods shall include the following: (1) The common name of the food, or absent a common name, an adequately descriptive identity statement. (2) If made from two or more ingredients, a list of ingredients in descending order of predominance by weight, including a declaration of artificial color or flavor and chemical preservatives, if contained in the food. (3) An accurate declaration of the quantity of contents. (4) The name and place of business of the manufacturer, packer or distributor. Due to limited desserts provided, manager will have a list of ingredients for each product, weight and manufacturer. Desserts will not be individually labeled. CFSM indicated during today's inspection that owners are working with the manufacturer regarding labeling.

The hot water in the wash compartment was only 104F. When taken from the faucet, the hot water is reaching a maximum temperature of 106F. The hot water temperature must be a minimum of 110F for warewashing. Increase the temperature on the hot water heater. The sanitizer is too high at 500ppm Quat. Contact service technician to calibrate. The Quat sanitizer must range from 150ppm to 400ppm.
